• Support School Funding - Family Income Inquiry Form | Financiamiento escolar de apoyo - Formulario de consulta de ingresos familiares

  Parents, please complete the Family Income Inquiry Form online. Families need to submit only one application for all of their children, even if their children attend different NYC schools. You can complete the FIIF in seven Languages on access NYC. Visit https://www.myschoolapps.com/ to apply. Paper applications are available at all NYC schools. If you have any questions please contact the office of Food and Nutrition services at (718)-707-4400.

  The Department of Health and Mental Hygiene has partnered with the Department of Education to issue this letter. Please review this letter regarding the COVID-19 vaccine and Flu vaccine as we get closer to the holidays, we want to make sure our PS/MS 15 community is a happy and healthy one. Happy holidays !
